Functional Needs Population

For temporary shelter, persons with functional needs would be evacuated by emergency workers to a congregate care center. If you are hospitalized or a resident of a special care facility, special arrangements to transfer you to another facility  are included in the hospital or facility’s emergency plan. If you have functional needs and might need assistance in the event of an evacuation, please complete the informational questionnaire below.
